Return Authorization (RA) Form

The Return Authorization form below is needed for sending a damper, pulley or other product to Innovators West for Return, Exchange or Repair. Please fully fill out the contact information at the top of the form and then use the dropdown box for the type of service requested. You must click on the red “Submit” button at the bottom once completed. Once submitted, the form will be emailed to the email address listed. There will also be a pop-up that will allow you to open the submitted form and print it out. Please include the form inside your package when you ship your parts in so we know what needs done and whose parts they are.

NOTE – If the item is being sent in for Return or Exchange, the item will need to be sent to the place of purchase.

DO NOT send the item to Innovators West if it was purchased through a dealer. If purchased through a dealer, the item MUST BE sent directly to the dealer. Please contact the specific dealer to ensure that you are complying with their return or exchange policies.

Items purchased directly through Innovators West can be sent to the address at the top of the RA Form. Items sent in for Return or Exchange must be new, unused and un installed AND must be purchased within the past 30 days. A 20% restocking fee will apply.
